Intergrated Health Specialist
CurrentManage delivery of individualized and group education services to clients, utilizing a motivational and stage-matched approach. The focus is on supporting clients in their journey towards recovery and resiliency goals. Assist clients in identifying their strengths, needs, abilities, and preferences, particularly related to substance abuse and mental illness, and participating in the development of wellness plans using a holistic perspective, incorporating coaching on the Eight Dimensions of Wellness, and providing support to clients in achieving their goals and accessing public services. Collaborate with other members of the integrated health team including psychiatrists, primary care physicians, specialists (CPRC, Peer(s), ITCD, CSTAR, DBT, etc), nurses, dentists, NCMs, etc. regarding all areas of client wellness.• Facilitated access to public services such as financial assistance, housing, emergency aid, and food, ensuring clients had the necessary support. • Promoted independence by promoting education and skill-building in various areas, including daily living skills, employment/education, symptom management, communication/relationships, budgeting, and maintaining a residence. • Strengthened support by assisting clients in developing personal support systems, working with family members, legal guardians, and recovery communities, among others. • Offered individual assistance to clients in accessing necessary services, including accompanying them to appointments for medical or other health needs. • Conducted timely follow-ups with clients regarding appointments, completion of forms, and other related activities. • Re-engaged and supported consumers who had unexcused absences from the CSTAR program, actively promoting their recovery efforts. • Met service provision expectations based on client needs and adhered to company guidelines.
