Since commencing my career in hospital pharmacy at TUH, I have gained invaluable experience in a large variety of clinical and non-clinical roles (dispensary and pharmacy management, aseptic compounding and medicines information). In my current role, I am responsible for the delivery of pharmaceutical care to patients in the ICU, PACCU and HDU.My responsibilities include;• Attending daily consultant-led, multidisciplinary team ward rounds, where I adopt a patient-centred approach to pharmaceutical care planning for critically ill patients. • Reporting medication safety incidents to the hospital’s Medication Safety Manager and liaising with relevant stakeholders to identify causes of said incidents and potential solutions to prevent them from re-occurring. • Writing and reviewing of policies, procedures, protocols and guidelines, as relevant to my role in the Pharmacy Aseptic Unit and Critical Care Pharmacy Team, in addition to my role as clinical pharmacist representative on the hospital’s Peri-operative Committee. • Participating in the Pharmacy Dispensary weekend roster and the Pharmacy on-call roster, ensuring the timely supply of medications and the provision of medicines information/pharmaceutical care advice outside of the Pharmacy Department opening hours. • Completing and documenting medicines information enquiries as required by service needs and as they arise during the course of my duties as a clinical pharmacist and/or on-call pharmacist. • Training new pharmacists, pharmaceutical technicians and students on Pharmacy Department policies and procedures and on the provision of Pharmacy Department services, including dispensing and aseptic compounding services, as well as clinical pharmacy services (Primarily surgical and critical care pharmacy). • Preparing and delivering presentations/lectures to Pharmacy Department personnel and healthcare professionals external to the Pharmacy Department.

While working in a large acute teaching hospital, I was a member of a busy pharmacy department, where I completed rotations in dispensary, medicines information and aseptic compounding. I also contributed to the extensive clinical pharmacy service on a regular basis, under the supervision of my tutor pharmacist. My responsibilities included;• Supporting the pharmacy department dispensary service, including dispensing and checking medications, controlled drugs, blood products, parenteral nutrition and cardiopulmonary arrest resuscitation trays. • Compounding extemporaneous medicinal products where required, in line with Good Manufacturing Principles. • Completing a biweekly ward top-up of stock medications to two wards, an acute medical unit and acute medical assessment unit. • Obtaining medication histories, completing medicines reconciliation and endorsing drug charts, as appropriate, and conducting pharmaceutical care planning under the supervision of my tutor pharmacist. • Liaising with other healthcare professionals, in order to optimise the care of my assigned patients. • Counselling patients on the safe and appropriate use of their medications e.g. anticoagulants, and on the effective use of devices e.g. inhalers. • Presenting content on a rotational basis and participating in a weekly departmental education meeting. • Supervising and training new staff members and students on pharmacy policies and procedures and dispensary operations.

Memphis, Tennessee

While attending a highly-ranked college of pharmacy in the United States, I engaged with pharmacy students and professionals in a variety of practice settings and seized the opportunity to visit several leading clinical institutions, such as St. Jude Children’s Research Hospital. My responsibilities included;• Presenting a comprehensive overview of pharmacy education and clinical pharmacy practice in Ireland. • Attending clinical teaching sessions in a variety of subject areas, for example simulation-based assessment, respiratory pharmacotherapy, medical microbiology and pharmacogenomics. • Participating in pharmacist-led ward rounds in a variety of clinical settings, including a level one trauma centre, a neonatal intensive care unit and a paediatric infectious disease unit.

Southampton, United Kingdom

In UHS, I joined a multidisciplinary team, composed of approximately 90 pharmacists, including independent prescribers and consultant pharmacists. This introduced me to the key role of the pharmacist in this environment, while highlighting the potential for career development in the future. My responsibilities included;• Attending multidisciplinary ward rounds in a variety of different clinical specialities, including paediatrics, trauma, orthopaedics, oncology, respiratory and endocrinology.• Completing several different rotations in dispensary, aseptic services, regional medicines information, clinical trials and mental health.• Tending to the dispensary reception - Checking in patients’ to-take out (TTO) drug orders, downloading stock orders for each ward and assigning appropriate priority to each order. • Dispensing a variety of different prescription types, including inpatient, outpatient, TTOs and stock orders.• Recording patients’ drug histories, under the supervision of a medicines management pharmacy technician. • Gathering and analysing data for inclusion in review audit. Audit Title: “A review of blood glucose monitoring requirements in patients prescribed oral prednisolone.”

This placement provided me with exposure to an international pharmacy brand, offering a wide range of services, coupled with an extensive retail selection. My responsibilities included; • Advising customers on the safe and appropriate use of medicines, in line with the Boots Sale of Medicines Protocol. • Completing the Boots Healthcare Advisor Programme, which helped me to increase my product knowledge and improve how I communicate with customers. • Attending national study days, where I worked with other Summer Placement Programme students on several different case studies, improving my clinical knowledge. • Receiving the daily drug order and updating the stock files as appropriate.• Registering customers for the Boots Prescription Service and signing customers up for the Boots Advantage Card.• Completing continuing professional development cycles as per my training requirements. • Providing a high level of customer service when dealing with complaints and processing refunds where appropriate.

I gained a valuable insight into the role of the community pharmacist, while working in a busy community pharmacy setting with a fast-paced dispensary. This has helped me to understand the importance of being able to work in conjunction with other healthcare professionals, to provide a high-quality patient-orientated service.My responsibilities included;• Offering customers advice on the most appropriate OTC treatments for their ailments. • Dispensing of prescriptions under supervision, including all national reimbursement schemes. • Dispensing of controlled drugs under supervision, as well as making entries into the controlled drug’s register. • Compounding of extemporaneous medicinal products, for example; coal tar and Betnovate® RD Ointment. • Preparing and processing of monitored dosage system (MDS) boxes for individual patients and nursing homes using the Nu-Life MDS computer software. • Ordering of stock and ensuring adequate stock levels were maintained.• Counting and completing the weekly bank lodgement.

Obtained hands on experience in a pharmaceutical company with an elite Irish base and global reach. As a member of an integrated team, I assisted in ensuring the quality, safety and efficacy of the product, which aided me in understanding and adopting the concepts of good manufacturing practice, good documentation practice and good laboratory practice. My responsibilities included;• Testing and analysing of packaging materials as required by the production schedule, ensuring all testing was carried out correctly and efficiently, in accordance with written procedures and good laboratory practices.• Recording all applicable data and instrument usage on a Laboratory Information Management System (LIMS) and/or the appropriate instrument logbook. • Filing of documentation in an organised, chronological or batch sequence manner. • Collecting and storing retain samples for raw materials and finished packaged products, as appropriate.• Documenting and disposing of expired retain samples in a safe and efficient manner, in accordance with all relevant waste disposal procedures. • Completing training in safety and environmental procedures, such as fire safety and evacuation procedures.
