Client support includes business development of mobile and virtual nursing models, business management services for Medicare and Medicaid organizations, Managed Services Organization innovating nursing services in primary care and consultation on digital health platforms. Bauer has also led a multi-state Integrated Care Organization in performance in their Transitions of Care (TOC) pilot program for a dual-enrolled population, increasing member engagement in TOC and CCM and reducing readmissions by 30%.Bauer has had the privilege to serve high-risk and high needs populations from the ICU to the home for many years in multiple organizations, locations.
