*Managing a home for six men with traumatic brain injuries and other physical / psychiatric disabilities. *Provide direct care with support of the team to individuals in the home. *Conduct daily client assessment of physical, cognitive & mental well-being and safety.*Interface with parents / guardians, service coordinators and clients for quarterly status reviews and annual Individual Service Plans (ISP).*Coordinate medical evaluations, physcial appointments as needed working closely with agency nurse regarding medical issues and communicate frequently with a variety of doctors regarding the care of individuals. *Accountable for both residential and day services including all activities and vocational support.* Assisting individual(s) who demonstrate high risk behavior and challenging behaviors with developments of social skills, boundaries and establish behavior treatment goals.
