Care and support agency offering care and support to individuals with learning disability, physical health, mental health, autism and associated challenging behaviours

My role is working with a range of our most vulnerable adults aged 18years - 65 years, individuals who have a learning, mental or physical disability. My key Duties are to work to safeguard, protect and assess the needs of the individual's allocated to me, working alongside partnership professionals.I work in line with the requirements of the Care Act 2014, and assess Eligibility for care and support services. I implement care planning in order to meet individual needs; whilst avoiding risk adverse practice and applying an outcome focused approach. The Mental Capacity Act 2005 is a significant piece of legislation which is applied to my practice, to assess understanding and cognitive functioning. Where an assessment outcome is that a person lacks capacity to make a decision, whether this is significant to care planning or protection, a best interest procedure is followed; along with, where applicable an application to the court of protection and a consideration for deprivation of liberty. Whilst also respecting that an adult merely may choose to make an “unwise” decision, not complicating this with formal capacity. I carry out section 42 safeguarding enquiries, working together with other significant professionals, identifying and assessing signs of safety, implementing protection plans, risk assessments and applications to the court of protection where require. Applications may be seen before the courts in regards to family contacts, Any plan to remove an individual to a place of safety of whom lacks capacity and where there may be considerations in relation to human rights. The adult safeguarding threshold is referred to, as well as partnership working with significant others in implementing a strategy discussion to support ongoing required action and decision making. I assess and support placements to meet individual need, carrying out regular monitoring until permanency and stability is achieved.
