The Patient 360 uses claims data, scheduling information, and payer extracts to compile gaps in care, upcoming appointments, and other alerts into a single source, available to providers on demand via Tableau or through a weekly report, ready at a patient’s point of care • Developed an MS Access database linked to a customized SharePoint List that draws information from various data sources and creates an interactive database environment. It integrates into the Patient 360 process, and allows the bi-directional data flow of patient clinical data.
