work2future Foundation's 2015 Summer Jobs Program is aimed at assisting local foster youth and disadvantaged young adults gain invaluable work experience. As a Youth Case Manager,I actively connect with low-income and foster youth, aged 16-21, residing in Santa Clara County in hopes of recruiting them for the Summer Jobs Program. I meet with the youth one-on-one to discuss academic and career aspirations and use that information to help steer them towards positions that are aligned with their interests. I serve as a liaison between our motivated youth clientele and their new employers to encourage open communication, ensure a positive work environment, and provide support for all parties involved.I act as a mentor for young adults who have the desire to grow professionally but are lacking direction or know-how to get their foot in the door. Youth attend a work readiness workshop where they are able to build a resume, learn workplace etiquette, and enhance their interview skills.I serve as a work-site partnership developer, making connections with local companies and non-profit organizations who support our mission and are equally dedicated to helping youth in their community.

Supervises a caseload for clients requiring social services.Interviews clients in their homes, in the office, or via telephone to assess needs and problems; evaluates individual and family case information; determines appropriate methods of treatment; initiates long- or short-term treatment plans; makes referrals to other agencies, organizations, or divisions within the department when appropriate; works with service providers to meet client needs; and advocates on behalf of clients.Researches, evaluates, develops, and carries out, in cooperation with the client, financial and basic social treatment plans; assesses and evaluates progress in case plan.Makes home visits to assess housing and health conditions and family relationships; refers cases of suspected abuse and/or neglect to appropriate unit.Investigates referrals and provides services to clients where their physical or emotional welfare is involved; provides services for individuals incapable of protecting themselves.Coordinates service plans with supervisor, eligibility workers, service providers, and other agencies.Prepares case records, reports, and correspondence, including documents for the Courts, utilizes computer systems to input and retrieve information related to social service cases.Interprets and communicates policies, rules, and regulations of the department , service providers, and the general public within the scope of responsibility; confers with supervisor regarding caseload problems.Attends court proceedings and testifies in court.Arranges for guardianship or conservator-ship of clients.Participates in specialized activities depending on client population that include provision of adoption services, investigation of abuse and/or neglect. places clients into care facilities as appropriate; Performs comprehensive assessments of client's psychological, physical, social, and environmental needs; works with client and interdisciplinary team to develop a realistic service plan.

Provide guidance, training and mentoring to youth in day-to-day livings skills thatinclude facilitation of classes and house meetings, meal preparation, laundry training,housekeeping skills and educational needs. Supervise youth at the residence, either Safe Houseor Opportunity House. Responsible for all shelter conditions including cleanliness, supplymanagement and licensing regulations. Active participation at community events and meetingsas required. Maintain a professional attitude with youth and co-workers, follow and enforceagency rules. Be an active and positive member of the case management team
