Key Responsibilities:Acute Care- Manage the nutrition care of a 10-bed acute ward, ensuring individualised care plans for optimal recovery.- Recommend and implement appropriate supplementation tailored to patient needs.- Develop and implement nutrition interventions for a variety of patient conditions, including malnutrition, chronic diseases, surgical recovery, and pressure injuries.Community Health- Deliver tailored dietary advice for managing many different health conditions, improving patient outcomes through sustainable lifestyle changes.- Collaborate with the diabetes educator and GP to triage and provide targeted support for diabetes and other chronic disease management, enhancing patient self-efficacy.Aged Care- Design and oversee texture-modified diets, ensuring compliance with IDDSI standards and resident safety.- Advocate for HEHP diets to address malnutrition and support quality of life.- Coordinate with multidisciplinary teams, including nursing and kitchen staff, to optimise food service delivery and uphold dietary quality control.Food Service- Collaborate closely with kitchen staff to develop and implement meals that meet patients’ dietary requirements, including appropriate textures and consistencies.- Conduct IDDSI and general food audits to ensure compliance with standards and improve overall meal quality.- Drive quality improvement initiatives, integrating feedback to enhance food service efficiency and patient satisfaction.- Liaise with supplier representatives to evaluate products, ensuring they meet the hospital’s dietary and nutritional standards.
