Plan, develop, organize, implement, evaluate and direct the admission/discharge utilization review and swing bed programs of this facility. Assist in the development and implementation of admission/discharge, utilization review and swing bed policies and procedures. Experience using Interqual decision support tool to communicate with providers and other clinical professionals appropriate utilization review and swing bed criteria pertaining to patients/resident eligibility and other clinical treatment/procedures. Research current federal and state regulations, as well as professional standards to make recommendations for change. Perform admission, concurrent discharge planning, and retrospective reviews in a timely manner, referring cases for secondary review as necessary;Develop strong referral network and coordinate continuity of patient care with external healthcare organizations and facilities, including processing admission, discharge and LTC and Swing Bed authorization.
