This was a 6-week rotation in which I am assisting from the pharmacy side on hospital management. So far I am working on transparency rules with medication charges, trending hospital T-Code data pertaining to medication ADE’s and ADR’s to look for opportunities for pharmacy to improve. I am also utilizing excel to forecast out medication costs for the next year based off of last year’s spending month to month.

This was a 6-week rotation, where I rounded with first and second year resident teams on ICU patients. I gave recommendations and fulfilled drug information requests daily. I spent time in the coumadin clinic and oncology clinic as well. I also scrubbed in on several surgeries with the anesthesiology team, and programmed Alaris pumps to assist them with developing their own drug library to order from.

This was a 6-week rotation utilizing Sentri-7 on renal dosing, anticoagulation monitoring and daily IV to PO conversions. I also used Horizon Med Manager for looking up labs, medication history and discharge status. I preferred MAP-4 (medical access portal) because it had a more user-friendly presentation and was linked to the CPOE, so I did not have to separately log into that to do interventions. We manage patients in poverty and offer sliding scale payment towards necessary medications they could not afford to get anywhere else, including hepatitis C, opioid dependence, diabetes and many more.
