I am assigned to conduct monthly data verification and validation of reports received from all 12 districts, and conduct daily data surveillance (review) on priority diseases; COVID-19, Poliomyelitis, Yellow fever, Meningitis, Measles, and other diseases of Public health concerns. The monthly compilation, analyzing, and monitoring of immunization coverage for quick actions and corrective measures and data management (caption of data using updated reporting tools and results collation to the national/ program levels) of Expanded Programme on Immunization during National Immunization Days. Preparing monitoring and evaluation reports of various Districts and writing annual reports for dissemination for decision-making and documentation using the holistic assessment approach approved by the Ghana Health Service Headquarters. Training of health care providers through On-site Training Supportive Supervision (OTSS) on data management activities using rslog app; good record keeping on family planning and comprehensive abortion care (CAC) service provided. I also conduct data audits on case management, use of standardized reporting format, completeness and timeliness of reports, report writing, and proper documentation.

I review the questionnaire and recruitment tracking form for completeness and accuracy and visit the ANC site weekly to collect questionnaires and blood specimens. Ensure all sites have needed supplies such as rapid test kits, questionnaires, consent forms, 2ml Nalgene tubes, etc. Complete supervisor monitoring form and weekly site recruitment tracking forms. Transport specimens to district laboratory for centrifuge and transport specimens twice per week to the regional laboratory for storage. Share questionnaire and all forms with the study coordinator for data entry. I also report all incidents including protocol deviations, adverse events, sharp injuries and maintain regular communications with the study coordinator.

Conducting daily data/disease surveillance (review) on priority diseases; COVID-19, Poliomyelitis, Yellow fever, Meningitis, Measles, and other diseases of Public health concerns. The monthly compilation, analyzing, and monitoring of immunization coverage for quick actions and corrective measures. Mentoring of students and National service persons on data collection, its analysis, usage, and dissemination. Data management (caption of data using updated reporting tools and results collation to the regional level) of Expanded Programme on Immunization during National Immunization Days’ such as National COVID-19 Vaccination Day. Preparing monitoring and evaluation reports of various health facilities and writing annual reports for dissemination for decision-making and documentation using the score-card system which shows various coverage in color codes; red, yellow, and green. Training of health care providers through On-site Training Supportive Supervision (OTSS) on data management activities; good records keeping on case management, use of standardized reporting format, completeness and timeliness of reports, report writing, and proper documentation.

The following are some selected key activities conducted as Health Information Officer; Primary data collection and analysis, data verification and validation of reports in health facilities, and addressing issues of variations in DHIMS 2 and primary source documents. Regular data review on disease indicators with health staff and stakeholders to implement apposite response strategies to minimize the burden of malaria and other communicable/non-communicable diseases. Data management planning and implementation of malaria interventions; Long-Lasting Insecticide Treated bed nets mass distribution exercise, continuous routine distribution (Antenatal Registrants and Children due for Measles 2 vaccine), school-based LLIN distribution (Primary school pupil), etc. Management of measles Outbreaks investigation data and report writing in 2017 among the Fulani settlers in Atebubu. Conducting routine records review at all units of health facilities on priority diseases and compiling, analyzing, and reporting daily, weekly, monthly, and annually on surveillance activities. Data entries using Microsoft excel and District Health Information Management System Database 2(DHIMS2). Planning and implementation of Community-based Health Planning Service program (CHPS) activities including CHPS database development and management. Preparing Monitoring and Evaluation reports and Annual reports. Data manager for Expanded Programme On Immunization activities such as National Immunization Day (MOPV, IPV). Organizing and conducting performance review meetings (quarterly, annual) and preparing comprehensive reports for dissemination at the regional level. Development of a data collection tool using the ‘KoboCollect’ Application for integrated supportive supervision of health facilities periodically.

I was assigned to collect primary data such as patient registration to register clients (healthcare seekers). Recording final diagnosis into consulting room registers, recording medications prescribed by the clinician into medications record books, and other vital details of the client. Data verification and validation of patient’s records and addressing issues of variations in primary source documents (registers) and the patient’s folder. Completing and submitting of National Health Insurance Claim sheet for patient’s bills reimbursement and filling of patient folders into the appropriate shelves for easy retrieval during the subsequent visits.I also assist the district data manager in preparing monthly data validation and verification reports, and monitoring and evaluation reports for quarterly and annual.

Conducted end process independent assessment of National Immunization Days for Polio Eradication, Measles SIAs, and Yellow Fever campaign in Ghana by systematic sampling of households and eligible children for screening (immunization status using finger marking or history), and coordinating for mop-ups to achieve universal coverage in all districts visited.
