October 2010 to Dec. 2011 solo operation and maintenance of Medical Records Department in a 45 bed Long Term Acute Care Hospital. Create new 'hard'​ (paper) charts as patients admited; update daily with MD progress Notes, dictated documentation, hand written documentation, graphic support documentation, out-of-facility documentation merged to current patient records. Backlog filing was built up several months in arrears; caught up with this filing with the addition of hiring a part time file clerk. After Dec 2011, a third person was added to staff full time. This team and myself continue to present to coordinate and work in conjunction with each other's division of duties and daily responsibilities to keep department current and running smoothly, maintaining current patient charts as well as Archiving and placing into Secure medical records storage those records of discharged and deceased patients, having access to all at 24-hour notice. Currently still running the HIM Department as a 3-person team, 7 day a week access to records as needed. With the addition of EPIC software to our facility, education both myself and team members, nursing staff and all connected disciplines within the hospital, we are now electronic medical record. The Archiving and Storage process continues the hours of hands-on old physical charts. In light of current facility needs and staffing issues, one FTE presently relocated physically to the Front Lobby Reception and Information Desk, assisting all staff in support functions and Guest and Patient Services and Coordination with Case Managers, Social Workers, Discharge planning, and other clerical support within all departments of the facility. The wearing of "many hats" of this float position allows for computer, clerical and Reception/Information Desk duties to complement as needed, where needed.

History and Physicals, Discharge Summaries, Surgeries, Orthopedic Consults and Procedures, hospital progress notes, radiology reports, eye surgery reports, etc. Also charting and filing documentation to patient permanent files, Release of Information from facility charts to primary physician office documentation files for continuity of patient care. Emergency Room documentation processing, transcription, filing, analysis of completion of documentation in timely manner, physician reminders to complete documentation in required facility ByLaws and peer requirements, etc.
