Provide copies of medical records to patients, attorneys, insurance, Illinois State Disability Determination in accordance with HIPAA regulations and billing of same. Chart assembly and analysis of outpatient and inpatient records. Report analysis of physician’s dictation and signature on inpatient records and notification to physicians.
