Throughout this position, I worked with clients within the Department of Developmental Disabilities state system to develop and manage the New Jersey Individualized Service Plan to plan and preserve ongoing support while adapting to frequent changes in state legislature. I also created and maintained working relationships with clients and families to ensure effective collaboration while also maintaining the client personal goals. A main objective of this position is to complete monitoring through phone contacts, visits, and the annual New Jersey Individualized Service Plan meeting to secure support service delivery. Moreover, I also completed monthly deliverables for client monitoring and plan development in an efficient and timely manner and conducted crisis management to ensure the health and safety of clients and immediately resolved disruptions of services or created a plan of action.
