Current• Establish and build trusting professional relationships with member physicians, senior leadership and KOLs• Provide member physicians and other providers with information on their pharmacy utilization• Provide educational updates (e.g., new medications, guidelines, and FDA warnings) at monthly physician meetings. • Act as a therapeutic resource to member providers• Collaborate with Primary Care Providers to establish optimal medication regimens for patients• Perform… Show more • Establish and build trusting professional relationships with member physicians, senior leadership and KOLs• Provide member physicians and other providers with information on their pharmacy utilization• Provide educational updates (e.g., new medications, guidelines, and FDA warnings) at monthly physician meetings. • Act as a therapeutic resource to member providers• Collaborate with Primary Care Providers to establish optimal medication regimens for patients• Perform patient chart reviews to asses for potential interactions, inappropriate medications, and incorrect dosages• Establish and build relationships with Care teams to provide pharmacy resources to Community Network high-risk population• Provide direct medication education and consultation to patients though phone calls, office visits or home visits• Review and assess medication regimens, provided recommendations to patients to streamline their medication regimen • Participate in local site activities, including team meetings, curbsides with care team members and physicians, etc.• Actively participate in planning and growth development of the Integrated Care Management Program • Collaborate with colleagues on other job-related duties and projects as directed Show less
