Clinical Pharmacist
Current● Clinical pharmacist in a federally qualified health center (FQHC) primary care clinic focused on meeting the needs of Native American and underserved patients● Performs independent and co-visits with providers focusing primarily on chronic disease state management, such as diabetes, asthma/COPD, HIV/PrEP, hypertension, hyperlipidemia, medication-assisted treatment for substance use disorders, and cardiovascular disease● As the Diabetes Champion, considered the diabetes disease state expert on the clinical pharmacist team and the go-to reference for any diabetes-related questions, training, and information● As the Special Diabetes Program for Indians (SDPI) grant clinical coordinator, tracks annual metrics, develops diabetes-related programming, submits grant applications, and reports directly the IHS Division of Diabetes Treatment and Prevention (DDTP)● Precepts APPE students on their ambulatory care pharmacy rotations, allowing them to shadow and perform independent patient visits
