As a Medical Fitness Consultant in the healthcare industry, my passion is to promote, educate and advocate for physical activity and prescriptive exercise as a standard of care for the prevention and management of chronic disease in the adult and older population.More about Corley: For over 20 years, Corley Roberts, certified exercise physiologist, health coach, and healthcare quality professional, has dedicated her career to service. · Serving healthcare organizations to improve quality of care in reducing preventable hospital readmissions, improving processes in care and delivery, outcomes management, care coordination, cardiovascular and chronic disease care, patient and family engagement, and community development. · Promoting and providing evidence-based and guideline driven physical activity solutions that improve outcomes, and that address processes in care delivery, hospitalizations, and care coordination. · Serving individuals to improve quality of life and active living through behavior change health coaching and individualized exercise programming. Currently, Corley is Medical Fitness Consultant and Lead Exercise Physiologist for AFRESH, a grant-funded intergenerational community-living model for health and wellness under Urban Housing Solutions and Vanderbilt University School of Nursing, and other key partners and community members. The community complex is a type of ‘aging-in-place’ model and includes a Vanderbilt Clinic, resident housing, community center, social services, and health and wellness professional services. Corley serves on the core team and leads the development and execution of the physical activity component for low-income older residents residing in this community, individualized assessment and exercise planning based on chronic conditions, age, current fitness level, and social determinants of health. Corley also provides process and outcome measure development, data collection and analysis, strategies for engagement and quality improvement of the program. Prior to AFRESH, Corley worked as a healthcare Quality Improvement Advisor for Tennessee's state Quality Improvement Organization and CMS (Centers for Medicare & Medicaid Services), the Director of Quality for The American Heart Association, and other major healthcare organizations specializing in cardiovascular care and delivery, chronic disease management, healthcare quality, and medical fitness.
