• Develop comprehensive social history and psychosocial assessments• Involve patients (and families) in treatment planning• Monitor treatment plan with the patient on a daily basis• Attend scheduled treatment team meetings• Complete mandatory documentation within timelines established by applicable regulations• Provide linkage with appropriate community resources by maintaining knowledge of other systems,making referrals and identifying unmet needs (e.g., transportation, outpatient treatment referrals, etc.)• Develop comprehensive discharge plans beginning upon admission in collaboration with patients,patient’s families, multidisciplinary staff, referral agencies, and outpatient services• Conduct process and psycho educational group programming curriculum to include stressmanagement, coping skills, etc• Apply policies, procedures and state/federal laws related to working with children

• Keep the classroom safe and assist the students with continued learning in the absence of the teacher• Take attendance and document daily notes• Adhere to the curriculum and lesson plan assigned by the regular teacher• Comply with all school/state/federal regulations and policies in regards to children

• Provide behavioral, instructional, vocational and developmental assistance to students with disabilities• Chart behaviors, implement behavior management programs i.e. IEP's and behavior plans

• Provide direct care services, in particular, monitoring individuals with a mental and/or substance abusediagnosis and providing mental health support• Complete documentation• Assist with conflict resolution• Prepare materials for and conduct group
