Registered Nurse - Act Team
Current- Mental & Behavioral Health Nursing for patients with previous history of hospitalization due to severe mental health diagnoses - Registered Nurse for NYC ACT team working in shelters and the community to manage and coordinate care for individuals with high-acuity mental and behavioral health diagnosis. - Manages coordination of care, medication changes, treatment plan implementation and updates, working directly with the team psychiatric nurse practitioner (PMHNP)- Focus on maintaining wellness and stability, allowing for decreased hospitalizations and the patients to remain in the community - Managed and coordinated care for individuals with chronic medical conditions including (but not limited to) diabetes, hypertension, cardiac disease, high cholesterol, asthma, cancer, arthritis, and others
