CurrentManages a caseload of 70+ clients, evaluating home care needs and developing service plans accordingly Performs initial and annual assessments via home visits to determine client’s eligibility and need for financed home care and to monitor changes in financial and functional levelsConducts monthly review calls and counsels clients on home care issues and on maximizing their Support at Home Voucher, following up with providers as needed Educates clients and third parties about program requirements and procedures, including two provider mode optionsAdvocates on behalf of clients and collaborates with social workers and case managers for mutual clients to ensure their home care needs are metProvides conflict resolution between clients and home care agenciesDocuments via electronic health record all case coordination activity in a timely manner Refers to alternative community services as needed, including home-delivered mealsAttends continuing education classes and trainings to increase knowledge, skills, and attitudes related to case management, gerontology, and other areas
