CurrentHospital@Home is an Acute Service that bridges the gap between hospital discharge and referral to community services such as District Nurses or the Community Respiratory Team. Our team covers a vast area in Sussex, including Portslade, Seaford, Brighton, Uckfield, Hassocks, Haywards Heath, and Burgess Hill. Within this role, I have been responsible for managing three key pathways: OPAT/VAC Patients: Collaborating with the OPAT team, I care for patients stable enough to be at home but requiring IV antibiotics for extended periods. I conduct regular home visits, administering treatment either once or twice daily. Additionally, I specialize in Negative Pressure Therapy (VAC dressing) for post-surgical patients, many of whom are diabetics needing amputations. Respiratory Virtual Ward: I support patients requiring community-based oxygen therapy after hospital admission. Working closely with respiratory consultants and the Community Respiratory Service (CRS), I manage oxygen concentrators, ensure proper inhaler usage, monitor prednisolone and nebuliser weaning, and provide respiratory assessments and breathing exercises. Daily briefings with doctors and bi-weekly MDT meetings are essential to this role. Frailty Virtual Ward: Managing clinically stable patients with complex needs, I conduct full physical assessments and coordinate care plans involving poly-medication, social situations, and allied health services such as physiotherapy and occupational therapy. This pathway requires daily consultant briefings and bi-weekly MDT meetings. My role varies between patient visits and office coordination. During visits, I perform initial patient assessments and tailor care based on individual needs. When coordinating, I ensure all patient visits are covered, liaise with doctors, organize weekly patient allocations, and communicate with various services involved in patient care. At the end of each shift, I ensure staff safety and prepare for the following day’s activities
