• Contacts consumers and/ or their family members to schedule new referrals within 10 business days. Documents any attempted contacts and any pertinent updates to all consumers record as needed, which includes all interactions with consumers and their families. • Completes in home assessments to collect information from consumers & families to access which services through OPTIONS or caregivers support program they qualify for. Provide referrals and resources to other programs to consumers as needed. • Complete all paperwork related to assessments in a timely manner and upload to SAMS or PIA database. • Completes level of care assessments in different settings including hospitals, nursing facilities, assisted living facilities and homes to access consumers proper level of care. Refer to waiver program as needed. • Completes Reports of Need (RON) for consumers who are at possible imminent risk due to abuse or exploitation. Submit RON to Protective Services supervisor for review. Initiate protective service cases as indicated.
