    (6/07/17-8/05/18) I wrote behavior intervention plans (BIP), created documents for behavior contracts and data sheets for the adults with intellectual disabilities. I conducted formal and informal assessments such as the Child Autism Rating Scale (CARS), Assessment of Dual Diagnosis (ADD), Brief Psychiatric Rating Scale (BPRS), Diagnostic Assessment for the Severely Handicapped-II (DASH-II), Questions About Behavioral Function (QABF), Maladaptive Behavioral Assessment (MBA), informal antecedent-behavior-consequence observations, and helped to complete the psychiatric consultations that Shapiro's facility psychiatrist completed for each individual we served. I made psychotropic medication changes based off of the data as well as implemented the BIP's, psychotropic medication reduction plans, fading plans for other restrictive measures, and behavior contract changes. I ensured that the BIP's were carried out properly and monitored progress on and off campus. I also trained staff at the residential, work campus, and community placement sites in terms of how to best work with each individual and what their behavior intervention plan entailed. I wrote monthly behavioral progress for 31 adults with intellectual disabilities served, as well as documented their progress through quarterly medication reviews for each person taking psychotropic medications to collaborate with the facility psychiatrist to monitor/assess psychotropic medication use. I attended behavior intervention committee meetings and submitted progress to this committee for each individual. I counseled individuals on a daily basis, both formally and informally. I was a part of special team meetings, annual review meetings, and meetings with outside agencies including interviews for community placement and day work programs. I also awarded daily and weekly incentives to those I worked with.

For approximately a year of my time as an administrative assistant I, I managed approximately 100 staff and 66 individuals who resided on four units. In addition, I worked directly with the adults with developmental disabilities. I ensured there was proper staffing for three shifts, wrote various documents, and assessed staff and their interactions with the individuals we serve on a daily basis. I also assisted in training staff, conducted in-service trainings, helped to create positive relationships with outside establishments, monitored the individuals' progress while they were at work, checked for accuracy of documents, arranged various activities throughout our units for the individuals living there and installed agency policies, programs, and procedures. I helped to counsel the individuals with developmental disabilities to help them make good choices, grow on an emotional and mental level, and ensure that the least restrictive use of guidance of their behavior program is utilized. Additionally, I served as a professional member of the Inter disciplinary team, inputting information known about the individuals we serve.(7/16/15-6/6/17) As an Office Associate, Option 2, I had computer, typing, filing, correspondence, and organization duties. I typed most documents that needed to be placed in the individual's files including their Individual Service Plans, helped to check them for correct/completeness, filedthe documents involved in their master files, mailed out correspondence letters/information to guardians, and organized various documents. I switched positions because an Office Associate position could give me the slightly different schedule I needed at the time.

    (9/11 to 4/13) As a Qualified Intellectual Disability Professional, QIDP, I worked directly with adults with developmental disabilities and had case management responsibilities for 26 of the adults with developmental disabilities at a day, work site. I was a professional member of the Inter disciplinary team, performing assessments with the individuals we served in order to write Individual Service Plans while they were at work, with the use of various computer programs. I would assign services, deal with referrals, coordinate and lead meetings, track monthly progress, set behavior plans, collaborate with families and residential facilities, train staff, help to set up activities in and outside of the work site, create various activities to motivate the individuals we served while they were at work, and advocate for the individuals. I also helped to screen for those who could receive outside employment, encouraged and supported them once they received such employment, and helped some to gain independent living in the community. For a period of time I also was a Home Based Service Worker, working with one individual in their home and in the community to help them develop and strengthen skills and reach goals, including exposing that person to activities in the community they had not experienced before. (4/13 to 11/13) As the Lead QIDP, I supervised 22 staff, hired staff, entered new individuals into our building, and maintained a caseload of 15 individuals, in addition to the above duties. (11/13 to 1/14) Due to downsizing I went back as a QIDP with a caseload of 21 individuals.
