Maintaining a strong relationship with Business development team, Physicians, Nurses, Social work, Vendor and Medicaid consultants. Providing support for coordinators for new admissions to ensure that all aspects of the admission plan have been completed. Supporting the team and ensure efficiency of staff by working independently and as a team. Maintain a smooth and efficient operation of the office by serving as back up to other intake nurses or administrative staff, as needed.• Assists in all phases of pre-admission case management including obtaining all necessary documentation.• Managing and developing leads, Coordinating with field staff. • Participate in the collection and reporting of data regarding referral applications, admission, discharges and transfers within the program.• Contact Service Providers to update the status of pending patient referrals..• Assist in conducting intake interviews and writing intake summaries as needed.• Provide additional assistance with client admissions that present with difficult Medicaid and benefit issues.• Support coordinators in all necessary admission documentation, physician’s authorization, application for benefits, insurance authorization and coordination of benefits. Following up on No fault, Workers compensation, and Liability exhaustion, Ensuring policies/ diagnosis have no relation to Home care admission and ensuring reimbursement for the services. • Verifying Physician license/ NPI, Check for sanctions/ malpractice.• Generating and updating daily statistic reports.• Managing and scheduling Nursing home/ Orthopedic patient referrals and onsite assessments.
