Vision:Our program's foundation is designed to help create a healthcare team for patients to help minimize and manage their chronic health conditions. CardiacFITT understands that there needs to be an alternative solution to the existing healthcare management model that will innovatively address the barriers encountered in the traditional healthcare system.
