Communicate service needs to ILP Specialists. Ensure that youths strength and needs are identified, documented and followed up in WTLP.Ensure that progress is being made and documentation is complete to prove service is provided, needed, and addressing issue identified (CFSR)I ensure that the youths complete the Ansel Casey Assessment. At a minimum it needs to be completed at age 14, 16, and 171/2.Follow up with foster parents, group home staff and providers to ensure they are supporting the youth incompletion of their goals.Update the WTLP every six month.Court appearance with the youths. Intake paperwork for the youth entering the group home and also getting the youths registered in school.Maintain youth case files with a case load of sixteen kids, making sure that their medical,dental, and physical are up to date and their individual service plans are also up to date and is specific to the needs of each individual.As a case manager i also have to input data in shines and score by a specific date every month.
