Documentation Reviewer - Student Documentation Compliance

Accountable for reviewing and managing documentation-related statistics of each practice that include CPT code variances, non-billable charts, critical care, and MIPS measures. Responsible for 25 emergency medicine sites, 40 hospital medicine sites (including Intensivist and IP Pulmonology sites), and Skilled Nursing Facility sites. Educate providers regarding the completeness, consistency, organization, and accuracy of the medical record, reflecting the physician's clinical judgment and medical decision making with the overall goal of improving clinical documentation, coding, and reimbursement.

• Nonbill Resolution for Southeast Division Facilities• Create and Format Excel Reports• PDF chart documentation mark up and feedback to provide for the Directors for South Division• Communication with Billing Coordinators, Managers and Billing Supervisor(s) for chart resolution issues when necessary• Communication with the coding company (RTI) for Chart Resolution• Monthly calls with RTI for coding opportunities and education• Research and Analyze Excel Reports to provide chart examples to the Directors of Documentation Education from outliers for coding opportunities and education.• Analyze reports for Critical Care opportunities, Downcodes, Splints and Straps, EKG, Monitors, MIPS opportunities

• Supervision of Hospitalist/Emergency Department coders • Monitor daily work assigned to coders to ensure timely completion • Audit Charts for review of appropriate level of service• Survey charts for potential clients using Audit tool, Spreadsheets and Coding knowledge• Appeals to Medicare/Medicaid/Commercial Insurance • QA for Hospitalist, Urgent Care, and Emergency Department • Assigned CPT and ICD9 to charts • Extensive E/M coding and training• Physician Webinars • Coder Training • Train and oversee students from local career colleges placed for externship • Implemented the ability to obtain CEU’s through AAPC for the certified coders

• Lead Instructor for Medical Assistant, Radiology, and Medical Billing and Coding • Maintain inventory, order, and distribution of medical books to students • Plan and prepare lesson to deliver to student with interactive student involvement, Instructional courses taught include: Medical Terminology, Anatomy and Physiology, Human Diseases, Coding CPT and ICD9, Microsoft Office 2007, Medisoft Medical Billing Program, Electronic Health Records, Career Classes. • Exam Coordinator for NCCT • Direct contact with Managers, Physicians, and Administrators for student placement, and supervise students that are on Externship.
