Coordinate preparation of and conduct outreach activities to engage individuals and communities disproportionately impacted by health issues related to HIV, STIs, SUD, and HCV. Outreach activities may be performed during or outside of office hours and include, but are not limited to, community events, bar outreach and the use of social media platforms.In compliance with the WI DHS Counseling, Testing and Referrals (CT) Protocol, conduct confidential and anonymous CTR for HIV, STI and Hepatitis C Virus (HCV) focusing on individuals at high risk for infection.Assist clients of our syringe exchange program to access clean equipment for safe injection and overdose prevention.Assist clients to navigate internal and external systems and processes to access appropriate care and treatment for HIV, Pre- Exposure Prophylaxis (PrEP), Post Exposure Prophylaxis (PEP), HCV and Substance Use Disorders (SUD). Navigation services include but are not limited to facilitation of access to medical care, behavioral and mental health services, case management and/or social services, pharmacy services, health coverage and Medication Assistance Programs.Monitor outcomes of referrals and assist in evaluating patterns of barriers that clients may face when attempting to access resources and work toward resolution as appropriate.Conduct individual and/or group level educational sessions on the subjects of sexual health and SUD, including prevention and care of HIV, STIs, HCV, harm reduction and overdose prevention.Maintain files for each client participating in prevention interventions in compliance with State and Sixteenth Street protocols and complete required data entry in the appropriate platforms.Develop promotional and educational materials related to the services provided at the Prevention Center.AND MORE..

As a Youth Program Assistant @ Planned Parenthood of WI , I take on the task of assisting with the P2D’s ( Power 2 Determine) which focuses on educating youth about safe sex practices along with healthy relationships topics. We have just implemented our AQB ( Anonymous Question Box) which allows us to be able to put these boxes in schools, clinics, health centers, etc allowing said youth to be able to get those questions that they may be too afraid to ask anyone. This work also allows me to push forward my work in HIV prevention and stigma work allowing me to implement a preventive program. Targeting patients who may become back positive and need proper guidance. I hope this work allows us to be able to collaborate and bring better edification to this city and world. 🙏🏾
