Assist with management of the Clinical Department - Assure all nursing procedures & protocols are followed in accordance with established policies. Plan, conduct and participate in In-Service Education Programs. Complete performance evaluations.Implement, educates, coaches, validates & monitors the facility’s Infection Control Programs. Clinical Discharge Planner - in collaboration with Social Services. Care coordination with discharge planning, responsible for obtaining discharge orders, coordinating with patient & family members in obtaining appointments to primary physicians, specialists & encouraging patients to have preventative exams in the community to prevent repeat hospitalizations, & assisting with obtaining DME’s for safe transition to home. Attends care plan conferences - Collaborate with members of the interdisciplinary health team, patient & families to plan, co-ordinate, implement, & evaluate plan of care in the facility and safe transition to community.

Report to VP of Operations and responsible in submission of monthly metric reports related to Clinical Processes and outcomes.Visit each facility assigned - responsible for facility's clinical regulatory compliance, ensuring compliance with Clinical Programs, orientation, training of Director and Assistant Director of Nursing. Assist with recruitment & retention, staff training, and reeducation as needed.Support all facilities when Illinois Department of Public Health or outside Agency is in the building. Conduct routine visits for compliance with all state and federal regulations. Conduct mock surveys for State Survey preparation as needed.Responsible for oversight of documentation capture related to clinical components of Public Aid and Medicare Reimbursement ProgramAssist with facility risk management - review unusual occurrences and incidents of serious nature upon discovery and coordinates compliance with necessary follow up.

Responsible with planning, organizing, developing, directing & delegating (as appropriate) the overall operation of the Nursing Department in accordance with current federal, state and local standards, guidelines & regulations, and facility policies and procedures. Attends & actively participates in resident care plan as indicated. Provide support to all nursing staff related to Clinical concerns. Assure adequate staff on all shifts. Responsible for implementing and monitoring the facility's RAI process - MDS and Care PlansEnsure the completeness and timeliness of all MDS's, responsible for data function to ensure accurate and timely electronic transmissions of MDS Assessments.Ensure that the MDS Department maintains current care plan and MDS for all residents and that all documents pertaining to plan of care are consistent in accordance with current Federal, State and local standards, guidelines and regulations, facility policies as directed. Provide guidance to the IDT in terms of MDS related reimbursement and provide support to both nursing and therapy departments in relation to Medicare compliance.

Provide Nursing Care and Responsible with Med Pass, Treatment/Wound Care, Admissions, Discharges and Transfers of patients/residents to home, Acute care and/or other Health Care settings.Supervise Certified Nursing Assistants.Responsible for the documentation provided to patients/residents.Collaborate with IDT, Physician and family with regard to patient/resident plan of care.

Assumed the role of a Full time Scrub and Circulating Nurse assisting surgeons with surgical procedures.Collaborate with multidisciplinary team managing operating room assignments and preparing patients in relation to surgical procedure scheduled or emergency procedures.Evaluate patient's needs and ensure delivery of required services in the main OR department.
