Social work internship from August-December 2018 and hired from January 2019-May 2019.As intern to interim program director at the PCH location of HOME. Both positions had about the same duties which includes documentation of residents, creating and updating safety policies in each building, designing onboarding templates for new residents, creating required documentation checklist for onboarding. Duties also included meeting with prospective residents for the program, educating them on what the program can provide them, financial aide help, financial responsibilities, assistance with medicaid & medicare questions or issues, and following up with prospective residents after meeting for next steps. Connecting residents with other resources throughout the community such as senior center classes, free days for seniors throughout the city, mental health support, and assisting with finding transportation.Providing one on one counseling for residents (CBT and DBT focused). Holding group therapy sessions at each location with residents. Group therapy was focused on a holistic view of self growth and healing. Leading group art therapy, mindfulness exercises, and physical exercise classes for residents.
