Current• Support a caseload of individuals who meet functional and financial criteria to obtain and maintain needed services to remain safe and to prevent institutionalization• Create and revise personalized plans of service to ensure provision of Medicaid program and Medicaid Waiver services: review RN documentation and recommendations, discuss clients’ needs, strengths, and goals, and synthesize this information in order to formulate individualized service plans• Maintain monthly contacts… Show more • Support a caseload of individuals who meet functional and financial criteria to obtain and maintain needed services to remain safe and to prevent institutionalization• Create and revise personalized plans of service to ensure provision of Medicaid program and Medicaid Waiver services: review RN documentation and recommendations, discuss clients’ needs, strengths, and goals, and synthesize this information in order to formulate individualized service plans• Maintain monthly contacts with clients: evaluate for changes in clients’ health or functional status and for complaints with Medicaid service provision; review personal care agency service logs for accuracy; evaluate working status of One Time Password time entry devices• Report changes in client status or issues of concern in LTSS system and to local health department; approve short-term emergency increases in personal care hours for clients after a relevant change in functional capacity; revise and update plans of service as needed• Facilitate connection with non-Medicaid resources for clients as needed such as Food Stamps; coordinate with staff at other agencies as needed; follow up with clients to ensure continuity of Medicaid coverage• Complete concise and timely documentation in LTSS system recording activities performed with client or on client’s behalf with relevant activity codes• Correspond regularly with fellow Supports Planners and with Medicaid Unit Supports Planner Unit Supervisor to learn procedures and best practices; attend regular trainings with team and with Maryland Department of Health staff to obtain updates• Support office initiatives such as COVID-19 community vaccination clinics and Grab-n-Go Meal distribution by contributing in-person interpretation assistance Show less
