This was accomplished by combining medical treatment, Behavioral Health services, Social Determinants of Health and companionship all in one service. These services are provided where the patient lives, whether at their residence, automobile or homeless. Many socially complex/challenged patients have had a bad experience with the healthcare system and are not engaged in preventative care plus have a higher utilization of emergency services. This means more suffering for the patient and higher system costs. ▪ Provided leadership and implementation strategies for finding and engaging the most medically and socially complex/challenged patients.▪ Help to create programs that located and engaged patients, resulting in 90% patient engagement and 98% patient retention. ▪ Help to create patient services that lead to a 50% reduction in hospital utilization, saving the system over $10 million.▪ Identified and negotiated agreements with patient support vendors.

In the first non-profit “Director of Innovation” role in San Diego, I provided leadership, guidance and implementation strategies for innovation, new business ventures and partnership opportunities to help create social enterprises from current strengths that are aligned with the mission. The partnerships where based on recent law and regulation changes causing a shift from “Fee for Service” to “Value Based Care”. These partnerships combine medical treatments with the services that address the Social Determinants of Health, to treat the whole person; body and mind. The partnerships create new sustainable revenue streams for the Medical groups and CBO's, allowing CBOs to increase the amount of free services they provided to the people who need help.

Collaborated with the City Council, Planning Commission and participated in public hearings to gain consensus and obtain approval.▪ Opened the largest Certified Organic Vegetables and Berries greenhouse in San Diego County which saves on average 85% of water, 80% of fertilizer and only uses 70% less land and reduces fuel consumption and carbon output by about 90%.▪ Competing against over 5,000 local farmers, secured top rated stores to sell Home Town Farms produce including Whole Foods. Achieved the Whole Foods ‘Best Rating’ which only a small percentage of companies are awarded.▪ Chosen to be a guest lecture at UCSD, USD, UCLA, USC and other major universities in Southern California on ways of creating a new business model.
