Over the last 20 years, it has been a privilege to lead national advanced illness/family caregiving programs with leading health plans and health systems , the Blue Cross Blue Shield Association, Catholic Health Association's “Supportive Care of the Dying” initiative, AARP, Siegel /Gale ,Borders, and “Sesame Street.” Our team collaborated to create the Knight Ridder Tribune/Robert Wood Johnson Foundation's "Finding our Way: Living with Dying in America" newspaper series , which appeared in more than 160 publications, reaching over 7 million readers, and was translated into Spanish and Chinese. Furthering the objective to advocate for public access to hospice and palliative care services , it was an honor to serve on the board of the Robert Wood Johnson Foundation's "Partnership for Caring " initiative, The Community State Partnership and The National Association of Social Work Foundation. At present, we are developing a consumer app for family caregivers accessing palliative care options based on our proven communication algorithms. The app's educational content and structured conversations were shown to be effective in two Medicare demonstrations and three randomized control trials, and were successfully utilized in Kaiser Permanente's Advanced Illness Coordinated Care program.
