Forensic Assessment and Triage Unit (FATU) • Medium secure forensic inpatient unit working with patients under the Ontario Review Board (ORB) or on treatment/assessment orders who have incurred criminal charges in the community and are in hospital due to their mental health status• Part of a multidisciplinary team and regular contributor to daily rounds and patient briefings• Responsible for running groups and programs on the unit (i.e. cooking group, emotion regulation, mindfulness and recovery through activity) as well as functional/cognitive OT assessments

• Outpatient day program for the mood and anxiety population working specifically with people suffering from anxiety and depression• Duties include planning, implementing and facilitating group sessions covering various topics, including Life Skills, Stress Management, Assertiveness Training and Problem Solving • Responsible for completing intake assessments on all patients for admission from the program• Contributor to daily rounds, reporting on patient’s mental status, participation and overall progress • Responsible for managing a caseload of patients, duties include: providing individual therapy, resource finding and discharge planning

• Outpatient day program for individuals experiencing severe and persistent mental illness (mainly Bipolar Disorder and Schizophrenia)• Duties included planning, implementing and facilitating group sessions covering topics such as Life Skills, Social Skills and Goal Setting• Responsible for managing a caseload of patients, duties include: providing individual therapy, resource finding and discharge planning

• Specialized service providing comprehensive OT and neuropsychological assessments to individuals with mental illness in both inpatient and outpatient settings

• Delivered intensive case management to clients with mental illness, observing and monitoring each clients psychiatric status and sharing these results with a team on a daily basis• Dedicated member of a multidisciplinary team. Responsible for participating in daily rounds, making the daily schedule and updating monthly client calendars to ensure organization and efficiency• Independently ran weekly groups for the clients (social network group and nutrition/exercise group)• Responsible for writing accurate, detailed and timely notes on clients after each visit that include regular mental status assessments and observations

•Deliver case management to approximately 15 outpatient clients with severe and persistent mental illness in a fast paced environment, observing and monitoring each clients psychiatric status and sharing these results with a team on a daily basis•Dedicated member of a multidisciplinary team responsible for contributing to interdisciplinary rounds, liaising with team members in order to ensure best possible patient care•Independently administer, score and analyze standardized and non-standardized assessments to contribute to a full Life Skills Assessment write up {including an initial interview, kitchen assessment, and ILS (Independent Living Skills)} assessment to determine a patient’s functional status •Routinely attend psychiatry appointments with patients in order to gain better insight into their mental illness as well as their day to day functioning•Responsible for maintaining contact with patients on Community Treatment Orders (CTOs) as required by the law

•Independently managed a caseload of 10 clients with severe and persistent mental illness in a fast paced environment, observing and monitoring each clients psychiatric status and sharing these results with a team on a daily basis•Administered, scored and analyzed both standardized and non-standardized assessments, such as the OCAN (Ontario Common Assessment of Needs) and the GAF (Global Assessment of Functioning) as well as cooking, grocery shopping and kitchen safety assessments•Provided personal coaching, motivational interviewing and self-talk to help clients overcome various personal issues•Routinely advocated and supported clients with issues related to housing, employment, personal care, ADLs, structuring time, interpersonal relationships and general social support •Independently wrote treatment plans, with special attention given to observations related to client’s psychiatric status, personal goals as well as goals agreed upon by all team members. Findings were then presented to the team in group treatment planning sessions
