Work with a dynamic interdisciplinary Transition of Care team to improve health care outcomes for patients in the hospital, as well as in the community.

Serve as the Clinical Quality Program Manager for WellPoint/BCBSGA- dedicated to the State Health Benefit Plan of Georgia (670 K members). Develop performance improvement plans and oversee the clinical quality improvement activities/projects/programs. Work in collaboration with the clinical intervention and data analytics teams to design studies targeted at improving population health. Provide oversight to assure that qualitative clinical data analysis and client reporting outcomes are meaningful, easy to understand and actionable for the customer. Accountable for monitoring and making recommendations to improve state-wide HEDIS scores.
