Clinical Dietitian Ii
Current• Practices Medical Nutrition Therapy to assist with patient care in inpatient and outpatient settings• Utilizes OARS and Motivational Interviewing to promote positive behavioral changes with the goal of overall health and wellness • Collaborates within multiple interdisciplinary teams• Completes nutrition screenings, initial assessments, and follow-up assessments daily• Facilitates Quality Improvement tasks: tray accuracy, test trays, documenting weight & A1c trends, assessing the success rate of connecting food insecure patients with food assistance programs• Disseminates nutrition education on various disease states; such as diabetes, cardiovascular disease, wound healing, liver disease, and gastrointestinal disorders• Assists with development of Food and Nutrition Services Department policies• Delivers nutrition education presentations to target populations• Connects patients to other healthcare disciplines or outside organizations (such as Food Banks) to ensure patients are set up for success with their healthcare goals and needs
