Seasonal (Jan-April 23' & 24') Performs frequent case reviews, check medical records, speak with patients and care providers regarding treatment, and respond to the plan of care. They also make recommendations regarding the appropriateness of care for identified diagnoses based on the research results for those conditions.

Conducting telephonic wellness check-ins on the patients of your assigned practice, via phone with patients, practices, pharmacies, etc. to ensure that your patients are getting what they need between office visits.Utilizing motivational interviewing and solution-oriented approaches to empower patients to take charge of their own wellness in the context of their care plan.

Conducting telephonic wellness check-ins on the patients of your assigned practice or practices. You will spend 7-8 hours a day on the phone with patients, practices, pharmacies, etc. to ensure that your patients are getting what they need between office visits.Creating a personalized care plan for each patient you call on, in coordination with the practices and providers you support.Working to identify social determinants of health, gaps in care, and eligibility for assistance or other referral services for each of your patients. You will coordinate and collaborate with the practices' providers, clinical and office staff, and other community programs and resources to address all of these effectively.Utilizing motivational interviewing and solution-oriented approaches to empower patients to take charge of their own wellness in the context of their care plan.Making approximately 30-50 phone calls a day to yield the 12-16 patient interactions necessary to complete 20 minutes of CCM service for 10-14 patients a day.And doing all of the above while maintaining your patients' privacy, confidentiality and safety, and adhering to ethical, legal, and accreditation/regulatory standards at all times

Assisting patients medical concerns (counseling and educating), gathering objective and subjective data to serve the physician in patients care, monitoring all temperature logs and Procedures for the Federal and State Vaccine Program
