Service And Support Specialist
Current- Assesses individual needs for services, using a strengths-based approach, with consideration for important to/for, known/likely risks, working/not working, place on path to community employment, and integration in the community.-Develops Individual Service Plans (ISP), using Person Centered Planning. -Establishes budgets for services based on the individual’s assessed needs and preferred ways of meeting those needs.-Assists individuals, through objective facilitation, in making selections from the providers and services available.-Implements continuous quality review process of ISPs to ensure implementation of services and desired outcomes for the individual.-Connects individuals to community resources to meet assessed needs and/or to promote community life engagement.-Recommends policies and procedures for the operation of programs, and assures proper implementation in accordance with state and federal requirements.-Maintains a variety of records, reports, correspondence and other communication.-Consults with providers to improve quality of services, provides training and education, and may participate in provider reviews.
