- Acted as Community Lead for the West Lawn and Clearing neighborhoods for the Southwest region of the Chicago Department of Health’s Healthy Chicago Equity Zone initiative, leading a team of 12 public health ambassadors.- Helped boost COVID vaccination rates to over 77% in the West Lawn and Clearing through community outreach.- Conducted neighborhood health and needs assessments using Chicago Health Atlas to identify priority health issues.- Established and maintained a collaborative network of local stakeholders to address priority public health challenges within the community, such as identifying barriers, service gaps, and health disparities.- Developed and implemented hyper-local health and racial equity action plans to address identified health disparities.- Recruited and trained employees to distribute public health materials, improve health literacy, and enhance health care access by leading team meetings and professional development sessions.
