• Ensure all program data for all HIV service delivery points (HTS, PMTCT, ART, Laboratory, PSM, CMSMSS, etc) are validated, collated and keyed into the DHIS platform before the 10th of every new month.• Ensure that data entered into the DHIS is thoroughly reviewed and triangulated with a stipulated time, then shared with the PCT and other technical staff for review and inputs prior to (data) dissemination and use.• Ensure that monthly, quarterly and semester performance is strategically displayed via graphic charts at all program implementation units/facilities and state office as part of tracking project performance.• Instigate data review session at HIV service delivery points as part of ensuring quality of service delivery• Provision of program performance feedbacks on weekly and monthly basis to program managers, supervisors, state HIV program leads, facility heads, ART coordinators and facility M&E officers• Routine program monitoring Visits. I ensure that HIV program facilities are visited at least twice every month based on gaps identified from its data reviews. I also ensure that onsite mentoring and training of key service delivery personnel is conducted.• Data quality/Auditing: I ensured that data quality assessment were conducted every quarter in all ART comprehensive facilities where I supported.• Conduct Retention Audit Determination Tool (RADET) exercise across all ART comprehensive, PMTCT and drug pick up facilities in both Delta and Bayelsa States.• Support in training of different cardre of service delivery personnel on DHIS, RADET, Data quality Assurances, MIS documentation and other Data capturing tools.• Pioneer the implementation and utilization of electronic medical record (LAMIS & LAMISLite) for patient management across 19 ART supported facilities.• Ensure quality of care of PLHIV is achieved which is geared towards achieving UNAIDS 3 95s(HTS, ART & Suppressed VL)

• Support major state actors (DPRS, SASCP, SACA, HMB, NEPWHAN) in work plan development, training of facility and community volunteers and any other technical assistance required by the state.• Provide technical Support to the state M&E Associates and Data entry officers in carrying out their essential duties and responsibilities.• Build the capacity of Health care workers at facility level through routine on site mentoring and supportive supervisory visits on proper documentation and reporting system using appropriate national/GF tools for effective patient monitoring and management. • Through routine program monitoring, I ensured that all program data for all supported facilities are validated, collated and reported on the DHIS platform before 10th of every new month• Collaborate with other implementing partner in the state to achieve the “three ones” principle by working with local implementing partners, SMOH, SACA, LACA, State program staff to ensure that program monitoring and evaluation activities are appropriate for program tracking in the state. • Ensure that quality of care of PLHIV in care is achieved which is geared towards achieving UNAIDS 3 90s• Conduct routine DQA exercise in all ART comprehensive facilities and ensure that facility carry out internal DQA routinely. This is purposed to strengthen M&E system quality.• Conduct and validate bi annual RADET exercise in all ART comprehensive facilities. This is aimed at improving client’s retention and quality of care.• Carry out routine documentation audit on Global fund supported facilities data capturing tools and data reporting tools to ensure completeness of data reporting.• Conduct internal monthly meeting for all Data Entry Clerks for the purpose of reviewing performance, validating reported data and timely data entry into electronic reporting platform (DHIS).• Perform any other duties as may be assigned from time to time by the State Program Manager and the M&E manager.

• Work with the country office, state level staff & government counterparts and IAs to provide support and guidance on program/project monitoring and evaluation activities and on donor and government of Nigeria reporting requirements.• Monitor data processes from collection, collation, analysis, reporting to use and ensure the utilization of standardized monitoring and evaluation indicators and tools for consistency in measurement.• Active participation in State Continuous Quality Improvement Teams (CQI&T) and active representation during the Site Improvement through routine monitoring visits.• Participate in health system strengthening activities which include but not limited to on site mentoring at project sites and state levels, advocacy to state staff and facility management• Participate in sites assessment and activation activities and provide technical support as required• Plan, conduct and facilitate state and LGA (M&E) meetings and participation in the state M&E monthly and Technical Working Group meetings at the state level. • Active participation in periodic data quality assessments (DQA), Joint National Data Quality Assessment and data validation exercise at the facility and state level• Coordinate data collection, management, and analysis from research initiatives and public health evaluations, as needed, in conjunction with research teams• Perform other duties as assigned by the State Coordinator.

• Support in policy formulation, design impact evaluative assessment checklist to the design of CRY FOR HELP.• Planning and allotting workshop, capacity building training and travels for the Organization.• Coordinate logistics for smooth running of Outreach/field Operations.• Responsible for work plan and capacity development for the board consideration.• Responsible for preparation and preview of CRY FOR HELP paper works before its final review by the board • Preparation and presentation of monthly, quarterly, progress and activity reports.

• Worked with the state-level staff and government counterparts to provide support and guidance on program/project monitoring and evaluation activities on USG and GoN reporting requirements. • Provided support to the sites in the state, including interacting with site Program Managers, M&E focal points, M&E officers of SACA/SASCP, other Implementing Agencies and local research groups on needs and ensuring that these parties understand and can support these requirements. • Conducted routine monitoring visits to project sites, including the conduct of data quality assessments (DQAs) using official tools, assist in the preparation of monthly reports, and provide supportive supervision. • On a monthly basis, electronically analyzed (DHIS) state-level data and provide approved reports to State Office, National Office and local partners or other relevant parties.• Build capacity of local partners in project monitoring and with program staff and local partners, use program/project monitoring data to modify existing programs and design new programs.• Providing onsite mentoring to facility staff on data collections tools and Data Quality.• Keying into data management Software program data for subsequent analysis, identification and follow up of missing data.
