*Cal Medi Connect Project Case Manager, (April 2014-February 2018 ; October 2018-Present) with the project being a telephonic based partnership between the CMS(which administers Medicare and Medi-cal for the federal government), the State of California, And Healthnet, to show that with the addition of Behavioral Health Case Management (and Medical Case Management for the medical providers), that access to mental healthcare and medical care, could be coordinated to be easier for people and with better outcomes. Coordination with Medical plan case managers.*Aftercare Follow Up Case Manager on the FUH7 HEDIS Project. ( February 2018-October 2018) Telephonic Coordination with higher level of care care managers, utilization review and discharge planners at the hospitals, providers and members to ensure that members who went into a psychiatric hospital had follow up providers and appointments and that one of those appointments was within 7 days post discharge. The aim is to increase the percentages of those who follow up with appointments with mental health providers within 7 days of discharge; Coordination with Medical case managers. Coordination of efforts with other clinicians and supervisors to create pathways for members to more easily comply such as texting service to remind members of their appointments.
