Host Program Coordinator
Peer Supervisor
In conjunction with the Supported Employment Program (SEP) Manager, the Peer Supervisor oversees the operations of DESC’s Lew Middleton Drop-In Center and partners with DESC’s SAGE teams to provide robust recovery services. In conjunction with the SEP Manager, this position manages all aspects of the peer program, responsible for providing behavioral health peer and case management services to individuals enrolled in Medicaid. Responsible for team development and oversight with a focus on successful integration of peer’s support.Work alongside Peer Specialists to model and manage daily tasks, support in milieu management for the 216 Clinical Office and liaison with clinical teams to provide essential needs, increase crisis diversion and focus on harm reduction methods. Building a strong partnership with DESC’s 515 Clinical Office and Wall Street Clinical Office (soon to become Hobson Clinic) to support and guide Peer Navigator staff integrated into our SAGE Mental Health program at these sites. Well established in my own recovery and provide expertise that professional training cannot replicate. Using my lived experience of recovery plus skills learned in formal training, to deliver services in behavioral health settings to promote mind-body recovery and resilience. Peer Specialists and the Peer Supervisor provide consultation to promote a culture in which each client's point of view and preferences are recognized, understood, respected and integrated into treatment, recovery and community self-help activities. Services are provided primarily out of DESC’s 216, 515 and Wall Clinical Offices with outreach and engagement throughout King County on occasion. This position requires a high degree of coordination and collaboration with other DESC programs as well as outside agencies.
Certified Peer Counselor / Specialist (Cpc)
Participate as a member of a multi-disciplinary team providing outreach, engagement and support of on-going case management services to housed and homeless mentally ill adults.Utilize a Recovery based approach to assist consumers to reach their fullest potential.Facilitating access to food, clothing, hygiene services, medical care, community resources and services.Plan and facilitate consumer meetings with the purpose of eliciting consumer input regarding mental health program performance and to assure the consumer perspective is well-understood and reported back to the team and the Recovery Plan Review Group.Participate in the Bar Review Committee, crisis intervention and safety management tasks.Assist consumers with voicing their interests and goals through a variety of channels including: Goal Plans, Crisis Plans, WRAP and other methods of expressing individual preference for their recovery goals.Participate in the planning, organizing and co-facilitating of self-help and educational group treatment services for consumers served by the mental health program. These could be located at the drop-in or in housing program sites.Travel locally to outreach consumers in their homes, shelters, and in the community.Work with the case manager to stabilize and maintain consumers in housing. This encompasses a range of skills including modeling, teaching, coaching and assisting the consumer in the upkeep of their apartment.Work with case managers, employment staff, and clients, providing a role model and support for those clients interested in gaining and maintaining employment.Participate in psychiatric consultation, supervision, program meetings and in-service trainings; participate in clinical reviews and case conferences for consumers as scheduled.Comply with the agency's clinical accountability policies and procedures; maintain current and complete clinical and administrative records; participate in quality assurance reviews when assigned.
Residential Counselor
Interact with residents in the general milieu and common areas.Assist clinical staff in the coordination of services to residents; contact residents' outside service providers as necessary.Assist clinical staff in engaging residents through creative, resourceful strategies that build trust and confidence.Assist clinical staff in the initiation, facilitation and promotion of on-site activities, therapeutic support groups, outings and community meetings.Manage all building operations in the absence of other project and clinical staff as assigned.Operate all functions in lobby office, including checking visitors in and out, answering telephones and monitoring security systems.Maintain safety and security by monitoring all general access areas and enforcing project rules.Intervene in crises, respond to emergencies, and initiate action as required, including contact with emergency response systems.Write significant events involving residents and building operations activities in a daily log; read log daily.Assist with room turnovers, new resident leasing and orientation; rent collection and facilities inspections.Respond to resident complaints.Work cooperatively with clinical and maintenance staff sited at the project and with visiting providers; refer residents to clinical and other staff as necessaryInitiate appropriate response to maintenance requests.Participate in staff meetings and trainings.Assist with other property management functions as assigned.
