Panama City, Florida Area

As the MDS director I was responsible for completing MDS assessments on residents per RAI schedule and inputting information into computer than submitting it to CMS. I scheduled careplan meetings for residents as well as conducted them with careplan teams presence and involvement. I reviewed and updated existing careplans and initiated careplans for new residents or on former residents that their status had changed. I assisted with at risk materials and data collection. I helped review and maintain charts by state requirements. Worked closely with therapy department to monitor residents progression in therapy and help plan for discharge home upon successful completion. I issued ABN notices when Medicare A would no longer be covering residents current stay in skilled nursing facility and worked with resident and their families to help ensure a safe discharge home and assisted social services in this area as needed. Worked closely with Business Office Manager in regards to insurance policies and guidelines. assisted in obtaining prior authorizations for a skilled stay if it was a requirement. Reviewed referrals from hospitals and other facilities to assess information and decide if the person would benefit from our facility and services provided. Many other duties completed as required as well.

As MDS Assistant I helped complete MDS assessment based on schedule and input information in computer program. I conducted careplan meeting for residents with careplan team, resident, and residetns family. Updated careplans quaterly and as needed. Helped review and maintain charts per state requiremants.
