Serve as a Primary Care Manager for patients of all ages in a busy federally qualified health center. Provide acute care, wellness exams and chronic disease management for a diverse and challenging population.Treat infectious diseases, STIs among a diverse, urban population in a community health setting and counseled on prevention of STI reinfection.Prescribe pharmaceuticals, non-pharmacological interventions and treatment regimens as appropriate for assessed medical conditions.Refer patients to medical specialists and coordinated patient care with social workers, case managers and mental health professionals.Develop targeted health education and adherence strategies for diabetes, hypertension and hyperlipidemia medication regimens; educate on disease processes; and link patients to community resources.Take part in peer review processes, conduct chart reviews, and participate in quality improvement projects for four clinics serving predominantly culturally diverse population.Function as the only onsite medical provider at several locations
