I worked on the Your Healthcare Plus IL Medicaid Project. Our portion of the project was focused on bridging within certain conditions (COPD, HF, Asthma and DM). We looked for gaps in care based on pharmacy claims. We reached out to providers to notify them of the gap in care and we also notified our disease management nurses for support for the patient.
