Inpatient RN Case Manager

Utilizing advanced nursing and care management principles work within and across multiple systems to effect outcomes for a defined patient population. Clinical and financial stewardship. High accountability to both internal and external stakeholders. Using case management processes responsible for utilization/resource management, care progression and complex transition planning in the inpatient setting.

Collaborated with multidisciplinary team in care coordination and to create optimum transitions using patient/family engagement tools such as Eric Coleman’s Four Pillars Coaching and motivational interviewing. Complex transition planning to include psychosocial assessment, readmission risk assessment, overcoming barriers to discharge, advocating for palliative care, facilitating family care conferences, and integration of community resources. Knowledge of Medicare regulations and managed care. Effective utilization management using Milliman Guidelines. Point of care customer service and service recovery.

Staff RN on IMCU/telemetry unit, completed PCCN core, ACLS certified. Staff RN Emergency Dept, Charge RN on busy joint voluntary/involuntary psychiatric unit, led therapy groups, facilitated daily treatment rounds, patient flow/throughput, daily staffing and coordination with multiple external agencies throughout the state. Met with state legislator to advocate for increased state funding for inpatient psychiatric services.

Utilizing community health nursing principles, served as the only psychiatric RN in rural community. PACT team member, psych triage, medication and symptom monitoring, mental health assessments, and patient education. Care coordination with both internal and external providers, protocol development. Created wellness classes for patients living with chronic mental illness.

Emergency room staff RN in one of Washington state’s busiest ERs. Served on Quality Assurance Committee.

Staff RN in large academic tertiary and trauma center. Patient demographics included spinal cord injury, brain injury, stroke and complex neurological disorders. Co-facilitated brain injury support group.
