(current casual, former permanent part-time) York Region Newmarket Health Centre (LTC), Newmarket ON Rotating between Rehab, Behavioral and Dementia/Alzheimer’s Units, responsible for managing the care of up to 32 clients which includes administering medications, applying treatments, executing doctors orders, monitoring vital signs and ensuring their overall safety and well beingEffectively diffuse behavioural issues by redirecting clients while ensuring the safety of those around Lead and facilitate care conferences with family members and cross functional teams Responsible for maintaining necessary documentation through computer based charting and communicating with staff as neededResponsible for delivering 24 hour report to Personal Support Workers on each shiftPromote and advocate for client’s needs and quality of life Encourage feedback and support from staff and family membersAssist Personal Support Workers with feeding, bathing, transferring and other ADLs as required

Managing a high volume case load in Toronto, North York and Mississauga, oversee and monitor in-house client care by assessing clients homes both prior to and during Personal Support Workers visits to ensure high level care and safety requirements are being metTrain and educate staff, families and clients to promote understanding of safety and holistic care requirements Liaise with CCAC on client’s needs and conduct joint meetings as requiredPrepare daily reporting on each individual client Communicate with other health care professionals such as physiotherapists and dieticians to promote the well-being of each clientEscalate issues as required to address urgent client care needsPromote independence by encouraging clients to have involvement with their care on a day-to-day basis
