Communicates effectively with resident and families in regards to resident’s care needs. Develops service care plan and documents change in condition as required by state regulations. Monitors personal care staff by overseeing medication assistance and ADL’s needed.Supervises all personal care staff to ensure that they are up to date on their certifications and trainings. Coordinates monthly In-Service meetings with entire staff. Constant communication with case workers and physicians about care of resident.Conducts assessment with licensed nurse prior to admittance to Sarah Daft Home. documenting care: identifying progress toward desired care outcomes. creating a safe work environment by enforcing safe practice with procedure regarding patient care. Scheduling of all personal care employees Creating behavioral plans to help resident succeed in home environment. Encourage utilization of preventative care to reduce Emergency room visits Increase continuity of care by managing relationships with professionals hired within Sarah Daft and other home health/hospice agencies. Updating Nurse on significant changes of resident. Keeps inventory of medications and narcotics in safe/cabinets.Reorders medications through pharmacy and works closely with pharmacist to ensure medication do not run out for resident. Scheduling of resident appointments medical and non-medical. Scheduling and oversight of U of U clinic. Attends Salt Lake County, CRC, Flexcare, New Choices waiver meetings to keep up with industry standards.
