Communicates effectively with resident and families in regards to resident’s care needs. Develops service care plan and documents change in condition as required by state regulations. Keeps inventory of medications and narcotics in safe/cabinets.Reorders medications through pharmacy and works closely with pharmacist to ensure medication do not run out for resident. Scheduling of resident appointments medical and non-medical. Scheduling and oversight of U of U clinic. Attends Salt Lake County, CRC, Flexcare, New Choices waiver meetings to keep up with industry standards.
