This is a frontline practitioner role situated in Lewisham, South East London. The organisation is a Targeted Family Support Service which works in conjunction with Lewisham Early Help Services; providing support Services to those identified as meeting Level 2/3 criteria, and who without intervention will require Children’s Social Care Intervention. Thus, sets out to provide Early Intervention to vulnerable and at-risk children and their families. The service draws on Evidence-Based Models and the latest research to ensure a Multi-Disciplinary Approach is used to make a lasting difference to families.As a frontline practitioner, my main role is to provide rapid response to children and families in crisis. Whereby, I complete a range of need assessments, organise and chair MDT meetings (known as TAF reviews), produce reports, create and provide support plans for children and their families, and carryout the interventions which have been identified as necessary during the assessment period.Moreover, my role is rather extensive but often includes: offering Evidence-Based Family interventions; re-engaging children with education and supporting parents/carers into employment; tackling behavioural concerns with children and young people; empowering parents/carers to combat their children’s behavioural difficulties and provide the necessary parenting structures (including boundaries, routines and discipline); supporting children and young people with SEND and providing a support package for them, which often involves Multi-Disciplinary work; supporting with emotional wellbeing and mental health difficulties in both children and parents, and so on.
