Provide case coordination for individuals diagnosed with ID/DD, TBI, and substance use disorder in the New York City metro area. Assist individuals in accessing Substance Use disorder treatment, higher levels of care, and aftercare services in their community. Linkages grant coordinator is responsible for facilitating information sessions to providers on promising practices used to work with the target population and how to navigate barriers to treatment, developing and submitting quarterly reports of deliverable services, maintaining grant budget, and outreach to community organizations and treatment facilities. The PEIR Counselor facilitates EBP drug and alcohol prevention education curriculum groups for individuals with ID/DD and/or TBI who are at risk of misusing substances or developing a substance use disorder. Assists with PEIR recreation trips and encourage client participation in activities.

Provided group and individual counseling for clients enrolled in the OASAS outpatient, non-intensive Substance Use Disorder treatment program. Services available to adults and adolescents (14+) with dual diagnosis, I/DD, TBI, and individuals court mandated with DUI/DWI.
